Vaccine Update: Eligibility Expands

Vaccine eligibility is expanding to new groups in Oregon and Washington. To learn the latest:

Oregon: Phase 1B, Group 6 Eligible no later than March 29, 2021. More info is available at Oregon Health Authority.

Washington: On March 31, individuals who meet the Washington State Department of Health (DOH) Phase 1B Tier 3 and Tier 4 eligibility criteria can be vaccinated along with individuals who meet Phase 1A and Phase 1B Tier 1 and Tier 2 criteria. See this timeline.

Increasing Vaccine Confidence

To reach herd immunity, we can all do our part to address vaccine hesitancy and help instill confidence. Here’s how: lead with empathy, provide transparency, normalize questions, and deliver the key messages:

  • All of the COVID-19 vaccines are lifesaving, safe, and free to you.
  • Vaccination is the last piece of protection we need to put the pandemic behind us.

Coming Soon: COVID-19 Vaccinations in Our Clinics/MOBs

We are preparing to vaccinate patients coming into our medical offices soon – dependent on vaccine allocation by the state and federal governments. In addition, 5 MOBs will be hubs for members and non-members who are a focus for targeted outreach: North Lancaster, Chase Gardens in Eugene, Rockwood, Mount Talbert, and Westside Medical Specialties.

You’ll soon receive Information to answer questions and help guide KP members and patients.

Timing of COVID-19 Vaccine in Relation to Other Vaccines

The CDC recommends that the COVID-19 vaccines should be routinely administered alone with a minimum interval of 14 days before or after administration with other vaccines. However, there are situations where a shorter interval may be needed. New guidance outlines timing of COVID-19 vaccines in relationship to other vaccines and discusses recommendations for the patient discharging from the hospital. COVID-19 Vaccine Can Impact Screening Mammography

Following certain vaccines (including COVID-19), swelling of the lymph nodes under the arm may occur. This swelling is a sign the body is actively building up protection against infection as intended. Even though swollen lymph nodes may be a normal response after vaccination, it may also be a sign of other disease including cancer. If swollen lymph nodes are present during a mammogram, they often require additional imaging for evaluation.

Based on the above information, screening mammography is recommended before the first COVID-19 vaccination or 4-6 weeks after the second vaccination as applicable. It is important to note that this is a recommendation and not a requirement. Patients can decide to schedule their screening mammogram at any time.

An update to the scheduling questionnaire will be implemented to inform members at time of scheduling and reduce the potential for last-minute appointment cancellations. Kp.org has already been updated with this information. If you schedule mammograms on Epic, you will soon notice a new question within the scheduling questionnaire that will serve as a reminder to schedulers to inform the member of this information before the appointment is made.

It takes a village! During the mid-February winter storm, the Sunnyside Medical Center (SMC) Sterile Processing department could not use their sterilizers because the facility’s boilers and pumps were either down or at capacity supplying heat to the hospital. The Sunnyside Sterile Processing, Sterile Instrument Processing, and Logistics – Transportation, Distribution teams worked seamlessly together to ensure trays were sterilized and available. New! Changes to Quick Facts

We’re enhancing Quick Facts to bring you the most up-to-date, key data. Some of the changes:

  • Reporting on Johnson & Johnson single dose vaccinations.
  • Transitioning from reporting dose 1 and dose 2 vaccination rates and counts to reporting prime and series complete vaccination rates and counts (Johnson & Johnson single dose vaccinations are accounted for under the series complete).
  • Added capability to view vaccination rates jointly in aggregate by race/ethnicity AND age band.  For example, you can now compare rates among racial/ethnic groups for all members 65 and over. Soon, you’ll also be able to drill down into the race/ethnicity aggregation.

Oregon Health Authority Update: Eligible Counties to Begin Vaccinating Phase 1B, Group 6

Counties will have the opportunity to attest that they are ready to begin vaccinating individuals in Phase 1B, Group 6. Many counties have already done so. Refer to OHA’s Vaccine Information by County page to see if your county has submitted an attestation letter. Migrant and seasonal farmworkers who are currently out in the fields working are eligible.

Reminder: How Can Eligible KP Employees Get a Vaccination Appointment?

We want to make it as easy as possible for all eligible KP employees to get a vaccination appointment. The updated workflow below applies ONLY to eligible KP employees, clinicians, and dentists. (Volunteers go to Oregon COVID19 or Washington COVID19.)

  • If you are a KP employee living in Oregon, click this link to request your vaccination at theOregon Convention Center.
  • If you live in Washington and you have a health record number and a kp.org account, go to KP Member vaccination request.  If you do not have a health record number and no kp.org account, go to Non-KP member vaccination request.

KPNW COVID-19 Trends

From KPNW researchers, scientists, and physicians, effective March 18. Based on review of internal trends and external considerations our findings are:

  • Anticipated Surge: OR/WA outbreak still decreasing (R<1), but rate of decrease is slowing. 
  • We don’t see/anticipate any major changes in the short term, but leading indicators, transmission rate, and relaxed restrictions are consistent with stabilization rather than continued decrease.
  • Longer-term forecasts have much uncertainty due to new variants, slow rollout of vaccines, and potential re-opening of society, schools, economy.
  • Current modeling forecasts of future surges beginning later this Spring currently show low/moderate impact on hospitalizations, due to rapidly increasing vaccination of high-risk age groups.
  • Internal hospital census (ADC) weekly average was down 5% from last week to this week (11.0 to 10.4). Census was 10 on Mar 17. ADC forecast for this week is 8 by Mar 24 (w. error).
  • For internal hospitals the age distribution for admits and census shows a slightly lower average age in March compared to Jan and Feb. When we remove outliers, LOS is reasonably steady across Jan to March. We will continue to monitor.
  • Internal ICU use low but not zero. Last week’s rate was 11% (prior week was 13%). Rate based on 1 to 2 ICU patients and low COVID inpatient numbers. Internal ICU had 1 patient on Mar 17.
  • KPNW COVID+ weekly average of daily cases down 4% from last week to this week (30 to 29). Forecast is 6 daily cases by Mar 24 (w. error). The most recent daily case count was 21 on Mar 17.

Weekly Testing Turnaround Times

The average routine COVID-19 testing turnaround time (TAT) for the week of March 15 to 21 is 16.8 hours. Member communications, including kp.org and call center scripts, have been updated to inform members that TAT is usually within 24-48 hours so they can plan accordingly for travel.

OHA Announcement

Yesterday, Oregon Health Authority shared that Oregon expects to get more COVID-19 doses from the federal government in the coming weeks. That means everyone 16 and older will be eligible for COVID-19 vaccinations by May 1. The state will continue to make equity the center of our vaccine distribution efforts, ensuring that seniors, people with underlying conditions, frontline workers, and the Oregonians most vulnerable to COVID-19 have the opportunity for vaccinations as soon as possible.

Vaccinated? You Must Still Wear a Mask in Our Facilities

On March 10, CDC issued new guidance (see full recommendations) for people who are fully vaccinated against COVID-19 to gather without masks in small groups while in private settings. This CDC guidance does not apply to the workplace or health care environment.

KPNW employees should continue to follow established Infection Prevention and PPE guidance, even if fully vaccinated. This applies to all employees – those in clinical, administrative, and other areas. Even though you may be vaccinated, you can still carry the virus and risk transmission to others. Dr. Eric Poolman likens this to wearing a seat belt and having an air bag – double coverage! Because of PHI and the privacy of one’s personal choice, please refrain from asking about a co-worker’s vaccination status.

COVID-19 Vaccine Data and KP HealthConnect (KPHC)

Q: How long does it take for a patient’s COVID-19 vaccine to show up in KPHC?

A: That depends!

  • Oregon Convention Center vaccines come into KPHC every 24 hours.
  • State of Oregon Immunization Registry imports every 1 week.
  • State of Washington Immunization Registry imports 1 time per month.
  • VA employees who were given vaccines will not show up in KPHC. For these, the clinician or staff (MA, RN, or LPN) will need to enter as an externally administered vaccine. VA members’ vaccines WILL show up in KPHC from the state registries.

Q: A patient says they were vaccinated, why can’t we find it?

A: There may be a few reasons why you cannot see the vaccine (or can’t see it yet) including:

  • The immunizations need to be reconciled. Staff can complete this. (See attached job aid.)
  • They had a vaccine at the VA (see above). Staff can enter the vaccine info on the CDC card the patient was given into KPHC.
  • The outside records didn’t match up exactly with KPHC due to data entry issues or mistakes. Staff can enter the vaccine info on the CDC card the patient was given into KPHC as an externally administered vaccine.

Q: A patient brought their CDC card from their COVID-19 vaccine, what do I do with it?

A: Great! Staff can record it as an externally administered vaccine per the usual workflow.

Timing of COVID-19 Immunization in People with Altered Immunocompetence

KP national issued updated guidelines that summarize recommendations across specialties and clinical situations. Important to note that immunization of people with altered immunocompetence due to drug therapy should not be delayed when community transmission is high and any decision to alter treatment with immunosuppressives or delay immunization should be individualized with shared clinical decision making between the clinician and the patient.

Guiding Members and Patients: Pharmacy Vaccine FAQs

Can I get vaccinated outside of Kaiser Permanente?

Yes. If you are currently eligible, you can get a vaccination at no cost from any facility or at any vaccination event that has been approved by the state department of health – including your local retail pharmacy. We encourage members and employees who are eligible to get the COVID-19 vaccination wherever there is availability — even outside of Kaiser Permanente.

How will I know if my local pharmacy is part of the program? And how can I get vaccinated?

Individuals who are eligible for vaccination in their state and are interested in getting vaccinated at their local pharmacy should call or check the pharmacy’s website to find out if vaccine is available. Most pharmacy partners are using online scheduling systems to schedule vaccination visits for eligible individuals based on their limited available vaccine supply. For Oregon, the following chains are listed on the CDC program site:

  • Albertsons Companies, Inc. (including Osco, Jewel-Osco, Albertsons, Albertsons Market, Safeway, Tom Thumb, Star Market, Shaw’s, Haggen, Acme, Randalls, Carrs, Market Street, United, Vons, Pavilions, Amigos, Lucky’s, Pak n Save, Sav-On)
  • Costco Wholesale Corp.
  • Health Mart Pharmacies
  • Rite Aid Corp.
  • Walgreens (including Duane Read)

COVID-19 vaccine is free of charge for everyone. Participating pharmacies will bill private and public insurance for the vaccine administration fee. For uninsured patients, this fee will be reimbursed through the Health Resources and Services Administration’s Provider Relief Fundexternal icon. No one will receive a bill for a COVID-19 vaccine.

KPNW COVID-19 Trends

  • Anticipated Surge: OR/WA still in a good position in the short term with a decreasing outbreak (R<1).
  • We don’t see/anticipate any major changes in the short term, based on transmission rate and leading indicators. Things appear to be remaining stable, and we will continue to watch for short-term trend changes.
  • Longer-term forecasts have much uncertainty due to new variants, slow rollout of vaccines, and potential re-opening of society, schools, economy.
  • Current modeling forecasts of future surges beginning later this spring do not indicate the potential for equal or worse conditions than seen previously, especially regarding hospital admissions.

  • Internal ICU use rate up again. Last week’s rate was 13% (prior week was 1%). Rate based on 1 to 2 ICU patients and low COVID inpatient numbers. Internal ICU had 1 patient on Mar 10.

KPNW COVID+ weekly average of daily cases was flat (0%) from last week to this week (30 to 30). Forecast is 19 daily cases by Mar 17 (w. error). The most recent daily case count was 30 on Mar 10.

In the News: Patient Released from WMC after 97 Days

Last night, KOIN 6 News shared another KPNW success story. Kim Stoutt was able to return home after nearly 100 days at WMC recovering from COVID-19. New Approved Isolation Mask

We will soon offer a new isolation mask, the Dukal 1541, as we have worked through our available stock of BYD isolation masks. This product is being rolled out to all KP regions. Dukal 1541 masks meet the recommended CDC standard of ASTM level 1 for isolation masks. KP has validated the level of protection provided through testing via a third party and has confirmed they meet the CDC standard of ASTM level 1. In addition, our Interregional Infection Prevention & Control department has affirmed that Dukal 1541 masks meet the appropriate standards and fall in line with our Universal Mask Guidance.

Please note: Dukal 1541 masks may feel thinner than BYDs, which are ASTM level 2, but provide adequate protection in settings where we have been using BYDs. Also, although the packaging states “surgical mask”, Dukal 1541 masks should not be used in the ORs because they have ear loops. We will continue to provide approved ASTM level 2 surgical masks in those settings.

TB Screening and COVID-19 Vaccines

Have questions on timing of TB screening in a COVID-19 vaccine setting? New guidelines have been posted on InfoHub at COVID-19 vaccine and tuberculosis screening.

Updated Allergy Information on COVID-19 Vaccines

Please see the updates on contraindications and precautions now that we have a third vaccine, Janssen (Johnson & Johnson) under Emergency Use Authorization. A 15-minute observation period is required for all vaccines, and individuals with a history of allergies should be observed for 30 minutes. People with a contraindication to one of the mRNA COVID-19 vaccines should not receive doses of either mRNA vaccine (Pfizer-BioNTech or Moderna). However, people with a contraindication to mRNA COVID-19 vaccines may be able to receive the Janssen COVID-19 vaccine, and vice versa, provided precaution measures are taken.

Help Patients Get Prescriptions Quickly

There’s another option for members who want convenient, fast prescription delivery. Last week, the pharmacy began offering same-day and next-day prescription deliveries through the KP app and on kp.org. Delivery charges apply. This service is available for new prescriptions and refills EXCEPT compounded medications and OTCs. A valid credit card is required for payment. For more information, refer to the attachment.

When the COVID-19 pandemic began, Ron immediately paused volunteer opportunities to ensure the safety of our staff in the community. Shortly thereafter, the marches in protest of racism began. CDC Guidance for Fully Vaccinated People

On Monday, the CDC updated recommendations for fully vaccinated people in the non-health care setting. Please note that none of the updated guidance applies to health care provider staff, visitors, or patients in any of our health care facilities. The guidance is for the community at-large and includes a change in recommendations for what the fully vaccinated person may do in private, at medium- to large-size gatherings, and some activities when away from home. It also has some new guidance for fully vaccinated people who’ve been in contact with a COVID-19 case depending upon symptoms and whether the person is a resident or employee of a non-health care congregate setting or high-density workplace. After state and local public health officials have had time to comment over the next few days on these new CDC guidelines, we will update resources, practice support tools, and communications.

COVID-19 vaccines play an important part in protecting ourselves, our members, and our families from COVID-19. Arm yourself with the top COVID-19 vaccine details to guide our patients and members when it’s their turn to get vaccinated. The flier is attached to print and share in huddles and post on internal visual boards.

KPNW COVID-19 Trends

From KPNW researchers, scientists, and physicians, effective March 4. Based on review of internal trends and external considerations our findings are:

  • Anticipated Surge: OR/WA still in a good position in the short-term with a decreasing outbreak (R<1).
  • Longer term forecasts have much uncertainty due to new variants, slow rollout of vaccines, and potential re-opening of society, schools, economy. (No change)
  • Internal hospital census (ADC) weekly average was down 27% from last week to this week (18.0 to 13.1). Census was 10 on March 3. ADC forecast for this week is 6 by March 10 (w. error).
  • Internal ICU use rate fluctuating. Last week’s rate was 1% (prior week was 8%). Rate based on 0 to 2 ICU patients and low COVID inpatient numbers. Internal ICU had 0 patients on March 3.
  • KPNW COVID+ weekly average of daily cases was down 22% from last week to this week (37 to 29). Forecast is 21 daily cases by March 10 (w. error). The most recent daily case count was 24 on March 3.

Weekly Testing Turnaround Times

The average routine COVID-19 testing turnaround time (TAT) for the week of March 1 to 7 is 15.4 hours. Member communications, including kp.org and call center scripts, have been updated to inform members that TAT is usually within 24-48 hours so they can plan accordingly for travel.

COVID-19 Vaccine Update: What You Need to Know to Guide Our Members

Patients and community members continue to struggle to keep up with eligibility and scheduling changes. Here’s what you need to know when guiding family, friends, members, and patients. More details are in this chart.

Kaiser Permanente is an approved COVID-19 vaccine provider in Oregon and SW Washington, meaning we administer the vaccine to Kaiser Permanente members and non-members according to the phases set by the Oregon Health Authority (OHA) and Washington State Department of Health.

  • Vaccine supply is allocated weekly by the Oregon Health Authority and Washington State Department of Health. Current demand by eligible groups outpaces supply. Not everyone who is eligible will be able to get vaccinated right away.
  • Based on current supply, it is estimated that existing prioritized populations will take a few months to vaccinate. We expect vaccine supply to increase over time so all adults can get vaccinated by late 2021.
  • You can get a COVID-19 vaccination from any state-approved vaccine provider at no cost.
  • Vaccine supply at our medical office buildings is limited.
  • We are encouraging eligible members in both Oregon and Southwest Washington to get the COVID-19 vaccine wherever there is availability, and at no cost.

Quarantine Update

We are resuming quarantining health care providers (HCPs) based upon risk assessment. HCPs who are contacts of COVID-19 with highest-risk exposures will be quarantined and off work. They may return to work after 7 days if a negative test 5 days or later beyond exposure is negative. HCPs who are 14-90 days from completing an approved vaccine series or have had COVID-19 infection in the past 90 days do not need to quarantine – however, testing is still recommended at day 5 after exposure. Examples of highest-risk exposure are having a household member with COVID-19 or having a high-risk workplace exposure. Please call Employee Health in these circumstances at 844-951-2060. COVID-19 Vaccine Update: Changes at the OCC All4Oregon Mass Vaccination Site

This week, we took 2 steps to ease the vaccination process at the Oregon Convention Center:

  • New Space: After being closed Sunday and Monday for the move, the site reopened today in a new, larger, more convenient space on the first floor, in the exhibit halls located near the entrance from the parking garage. This will significantly shorten the distance patients need to walk, which will especially benefit seniors and those with mobility issues.
  • New Appointing Process: To help reduce challenges faced by those trying to schedule a vaccine appointment at the OCC, the Oregon Health Authority will now use the Get Vaccinated Oregon signup tool to fill appointments. Effective yesterday (Monday, March 1) the OHA will randomly select eligible residents in Columbia, Clackamas, Multnomah and Washington counties, who have registered at Get Vaccinated Oregon. All4Oregon staff will then call those individuals to schedule their appointment.

KGW recently aired a special report on the 1-year anniversary of the first COVID-19 diagnosis in Oregon, at Westside Medical Center. The KGW story shows the amazing dedication and teamwork of our staff and clinicians over the past year, and features interviews with Dr. Katie Sharff, Eric Cathey, RN, and Dr. Stella Dantas.

KPNW COVID-19 Trends

From KPNW researchers, scientists, and physicians, presented February 25. Based on review of internal trends and external considerations our findings are:

  • Anticipated Surge: OR/WA still in a good position in the short term with a decreasing outbreak (R<1).
  • However, we may be seeing a stabilization and potential increase in the transmission rate going forward.
  • Longer-term forecasts have much uncertainty due to new variants, slow rollout of vaccines, and potential re-opening of society, schools, economy.
  • Internal hospital census (ADC) weekly average was down 9% from last week to this week (17.9 to 16.3). Census was 11 on Feb 24. ADC forecast for this week is 8 by Mar 3 (w. error).
  • Internal ICU use rate up again. Last week’s rate was 8% (prior week was 0%). Rate based on 1 to 2 ICU patients and low COVID inpatient numbers. Internal ICU had 2 patients on Feb 24.
  • KPNW COVID+ weekly average of daily cases was down 12% from last week to this week (41 to 36). Forecast is 33 daily cases by Mar 3 (w. error). The most recent daily case count was 33 on Feb 24.

Weekly Testing Turnaround Times

The average routine COVID-19 testing turnaround time (TAT) for the week of February 22 to February 28 is 16.2 hours. Member communications, including kp.org and call center scripts, will continue to inform members that TAT could be 48 to 72 hours so they can plan for travel.

Marking 1 Year of COVID-19\

A year ago, Westside Medical Center admitted Oregon’s first COVID-19 patient – and our lives, our families, our workplace, and our communities changed forever.

Since that diagnosis was made on February 28, 2020, we’ve witnessed the toll of COVID-19 – the trauma, turmoil, economic devastation, and heartbreaking statistics, including that, across the United States, more than 500,000 people have died.

It’s been a sobering year, but we’ve seen light within that darkness. And that light is all of you – our entire KPNW family of health plan and hospital employees, NW Permanente staff and clinicians, and Permanente Dental Associates dentists and staff. Continue reading the message here.

Surgical Services Update

The surgical and procedural services scheduling moratorium of non-medically urgent cases has been lifted. This moratorium was announced on November 23, 2020 and extended to February 28, 2021, affecting hospital-based surgical and procedural rooms, as well as limiting ASC capacity due to staff redeployment. Starting March 1, elective cases are scheduled in open rooms at all sites.

WMC COVID-19 Unit Update

Plans to dismantle and deep clean the dedicated COVID-19 Unit at WMC are underway. We have paused transfers of COVID-19 patients to WMC from other hospitals (including SMC) until we have fully developed our 1st and 2nd floor COVID-19 areas. The conversion is due to be completed no later than March 5. Please contact a member of the hospital leadership team if you have questions.

Tools to Help Members Navigate the Vaccine Maze

New resources and outreach efforts have been created to help you better serve our members and direct them to the right source for vaccine information in their community.

As vaccine supply is limited at our medical offices and each county manages supplies differently, we are encouraging our eligible members to get the COVID-19 vaccine wherever there’s availability. Information for directing Oregon and Washington residents on vaccine eligibility and locations is easily viewable in the state and county resource charts, also in the attached PDF.

Here are some other tools and work happening to help support our members:

  • The Kaiser Permanente Northwest Vaccine Q&A (also in the attached PDF) now includes state and local answers to questions such as: Am I eligible to get the vaccine; and, if not, when? I’m eligible, how do I get an appointment? I’m not eligible, can I sign up to be notified when an appointment is available?
  • The KP Vaccine Hotline provides a recorded message with general info: 1-855-550-0951.
  • If someone has questions after listening to the recording, they will be connected to representatives at the KP National Contact Center who can provide Oregon- and Washington-specific answers and do warm transfers to NW Contact Center representatives.
  • KP.org/covidvaccine/nw is the primary source for member information on COVID-19.
  • KP.org/coronavirus provides “one KP” updates on testing, treatment, research, and more.
  • Targeted member emails are sent each Friday, providing localized information and resources for getting a vaccine.

Governor Kate Brown extends Oregon’s state of emergency order until May 7

Oregon Gov. Kate Brown on Thursday extended Oregon’s state of emergency declaration for another 60 days. It will continue until May 2. The declaration legally authorizes Brown’s COVID-19 executive orders and the Oregon Health Authority’s (OHA) health and safety guidance. Extending the declaration allows those orders to remain in effect.

Member Vaccine Update: KP.org “Waitlist” Closing, New Tools for Members Coming Soon

The COVID-19 vaccine “waitlist” on kp.org – which served as a notification of vaccine eligibility – will be closed effective Friday, February 26. We will be contacting people on the waitlist to provide them information on next steps and additional resources.

This change is happening as Oregon and Washington have launched new tools and resources to help inform residents on eligibility, locate vaccine sites, and learn how to register for an appointment in their community. As vaccine supply is limited at our medical offices and each county manages supplies differently, we will be encouraging our eligible members to get the COVID-19 vaccine wherever there’s availability. Members and non-members can get a COVID-19 vaccination from any state-approved vaccine provider at no cost.

OCC Mass Vaccination Site Moves into New, Larger Space

On Tuesday, March 2, the All4Oregon Mass Vaccination site operated by Kaiser Permanente, Legacy Health, OHSU, and Providence will migrate into a larger and more easily accessible space at the Oregon Convention Center. Days and hours of operation will also be adjusted.

In just over a month since opening the site, we have outgrown the current space in the Oregon Ballroom. Moving into the Exhibit Hall will make the vaccination process easier and more convenient for patients arriving for their appointments. And, it will allow us to better meet the needs of special populations of people, such as seniors. To move into the new space, we will close the site on Sunday, February 28 and Monday, March 1 and have reached out to reschedule appointments for those two days.
